What is an altered patient?

Clinical issue. You are consulted for an “altered” patient whose dysfunctioning cognition and behavior are complicating the patient’s clinical care. Or, perhaps you are an outpatient therapist who notices that your patient’s attention is waxing and waning, and is drowsy during your interaction.

Is amitriptyline metabolized by CYP2D6?

Amitriptyline is heavily metabolized by CYP2D6. Nortriptyline (e.g., Pamelor) Nortriptyline is a tricyclic antidepressant. Some studies found amitriptyline to have a higher anticholinergic burden at a given dose compared to nortriptyline (Chew et al., 2008).

What is contact with the present moment?

Contact with the present moment is the process most similar to mindfulness, as it emphasizes being present with your thoughts and feelings without judgement . This better allows the client to use the processes of acceptance and cognitive defusion. Dr. Hayes describes it as “pure awareness, period.”

Does CBT have the same efficacy as ACT?

ACT and CBT had equal efficacy for depression. There are a number of studies comparing ACT and CBT, which we provide below. In general, the studies showed equivocal efficacy for both types of therapy.

Is there a difference between ACT and CBT?

In an RCT of 87 patients with an anxiety disorder, there was no significant difference between ACT and CBT treatment after the 12 sessions.
